Hotel or Organization or Enterprise Accounts: Libraries: Enrollment Forms

The purpose of this page is to create a reusable set of enrollment forms for your Hotel or Organization, and make them available within your bundles and events.

For Enterprise Accounts, the purpose of this page is to create a reusable set of enrollment forms to be made available to hotels affiliated to the enterprise organization as Sister Properties.

Creating an Enrollment Form

  1. Click the link at top of page to create a new enrollment form.

  2. Enter a name for the enrollment form and customize it as you like.

  3. Click the Save button to save the form and return to the enrollment form library.

You will see the new enrollment form in the list.

Turning on an Enrollment Form

To make an enrollment form in your library available to bundles and events, you must first turn it on. Click the ON/OFF switch to ON to make it available to bundles and events. Setting the switch to OFF will make it invisible to bundles and events.

For Enterprise Accounts, to make an enrollment form in the enterprise library available to the established Sister Properties of the organization, you must first turn it on. Click the ON/OFF switch to ON to make it available to hotels to select for their bundles and events. Setting the switch to OFF will make it invisible to the hotels.

TIP: Turn ON an enrollment form in your library once you are satisfied with your edits. This will ensure that only enrollment forms that are ready for use will be available to bundles and events.

IMPORTANT: To actually use the enrollment form on your event website so that attendees can complete it, you must enable it in the event.

Editing an Enrollment Form

  1. Click the edit icon () next to the enrollment form you want to edit.

  2. To return to the library once you have finished your changes, click the Save button.

Copying an Enrollment Form

Copying an enrollment form allows you to reuse it as the basis for a new form in your library.

  1. Click the copy icon (), enter a unique name for your website, and click the Save button.

  2. To return to the library once you have finished your changes, click the Save button.

You will see your newly added enrollment form in the library.

Deleting an Enrollment Form

Click the delete icon (X) next to the enrollment form you want to delete.
